Succinylcholine neuromuscular block in the myasthenia gravis patient.

چکیده

The decrease of “functional” endplate receptors in MG can decrease the response to the chemical transmitter acetylcholine, as well as to the depolarizing muscle relaxant succinylcholine. In contrast, the decreased “safety margin” results in a marked sensitivity to nondepolarising relaxants. In normal patients, the wide “safety margin” may explain the slow onset of nondepolarizing block, as well as the rapid onset of succinylcholine depolarizing block. In myasthenic patients (Fig. 1), the decreased “safety margin” not only potentiates the neuromuscular block of non-depolarizing neuromuscular lockers, but also speeds its onset of action. In contrast, this can decrease the depolarizing action of succinylcholine, with a subsequent resistance and delayed onset of action4.

Safety and Efficacy of Electroconvulsive Therapy in Myasthenia Gravis

Myasthenia Gravis is autoimmune disorder of neuromuscular transmission. In some cases, psychiatric complications aggravate its course. Myasthenia gravis (MG) has always been a challenge for anesthesiologists and psychiatrists when the use of electroconvulsive therapy (ECT) is required, especially when succinylcholine is used together with acetylcholinesterase inhibitors. We describe a 76 years ...

Sugammadex as a reversal agent for neuromuscular block: an evidence-based review

Sugammadex is the first clinical representative of a new class of drugs called selective relaxant binding agents. It has revolutionized the way anesthesiologists think about drug reversal. Sugammadex selectively binds rocuronium or vecuronium, thereby reversing their neuromuscular blocking action. Due to its 1:1 binding of rocuronium or vecuronium, it is able to reverse any depth of neuromuscul...

A Case Report of Congenital Myasthenia Gravis Presenting With Respiratory Distress

Congenital Myasthenic Syndromes (CMS) are rare inherited disorders characterized by dysfunction of neuromuscular transmission at the neuromuscular junction. Most patients with congenital myasthenic syndromes present in the infancy. Major symptoms of affected individuals include weakness and fatigue during the first years of life. Patients may show hypotonia, facial weakness, swallowing difficul...

[Total intravenous anesthesia for thymectomy in a Myasthenia Gravis patient: case report.].

BACKGROUND AND OBJECTIVES Patients with neuromuscular diseases, as Myasthenia Gravis, react abnormally to anesthetics, depending on the technique and on the drugs. This report aimed at showing a case of Myasthenia Gravis patient submitted to thymectomy under total intravenous anesthesia with propofol and remifentanil. CASE REPORT Female Myasthenia Gravis patient, 52 years old, 72 kg, submitte...

Bradycardia, hypotension and bronchospasm following remifentanil-propofol in a myathenic paitent treated by pyridostigmine--a case report.

Myasthenia Gravis is an autoimmune neuromuscular disorder of skeletal muscles due to the presence of acetylcholine receptor antibodies. Myasthenic patients are resistant to depolarizing relaxants such as succinylcholine but are very sensitive to non-depolarizing muscle relaxants1. That’s why, narcotic-based technique for induction and maintenance of anesthesia without muscle relaxants has been ...
